Choptank Community Health

DENTON — Choptank Community Health, a nonprofit Community Health Center based in Denton, was recently awarded a Quality Improvement grant of more than $148,000 from the U.S. Department of Health and Human Services.

The grant was awarded in recognition of Choptank’s efforts to expand access to comprehensive care, improve health care quality and outcomes while increasing comprehensive care delivery in a cost-effective way. The Choptank Model of Care addresses health disparities by advancing the use of health information technology and delivering patient-centered care to the residents of the Mid-Shore region.

Choptank was recognized as a Health Center Quality Leader, which puts them in the top 30 percent of community health centers nationwide that achieved the best overall clinical performance on specific quality measures. Choptank performed in the top quartile nationwide in clinical quality measures such as adult weight assessment and counseling, diabetes control, childhood immunizations, dental sealants for children, and counseling about activity and nutrition in childhood.

“Community health centers provide coordinated, comprehensive, and patient-centered care to millions of Americans,” said HHS Deputy Secretary Eric Hargan. “They have a track record of delivering quality care at significantly lower cost and are vital partners in our movement toward a health system that delivers quality, affordable, value-based health care for all Americans.”

In 2017, more than 27 million people relied on a Health Resources and Services Administration-supported health center for affordable, accessible primary healthcare, including one in nine children 17 years or younger; one in five rural residents;one in three people living in poverty; andmore than 355,000 U.S. veterans.

This award recognizes that Choptank continues to make a positive impact on patients, families, neighbors and the community as whole.”

Choptank Community Health has been providing primary medical and dental care to residents of the Mid-Shore since 1980, with offices in Federalsburg, Denton, Goldsboro, Cambridge, Easton and St. Michaels. They also provide medical care in schools in Caroline and Talbot counties and preventive dental services in schools in Caroline, Talbot and Dorchester counties as well as to the Mid-Shore seasonal migrant population.
